সার্টিফিকেট, নম্বরপত্র কিংবা প্রবেশপত্র হারিয়ে গেলে করণীয়

সার্টিফিকেট হারিয়ে ভীষণ চিন্তায় পড়ে যান অনেকেই। কী করবেন, কিভাবে সার্টিফিকেট ফিরে পাবেন তা বুঝতে পারেন না। সার্টিফিকেট বা এ ধরনের মূল্যবান শিক্ষাসংক্রান্ত কাগজপত্র হারালে বা নষ্ট হয়ে গেলে ঘাবড়ানোর কিছু নেই।
প্রথমে যা করবেন
ঢাকা মাধ্যমিক ও উচ্চ মাধ্যমিক শিক্ষা বোর্ডের চেয়ারম্যান ড. ফাহিমা খাতুন জানালেন, সার্টিফিকেট, নম্বরপত্র বা প্রবেশপত্র হারিয়ে গেলে দেরি না করে এ বিষয়ে দ্রুত পদক্ষেপ নেওয়া উচিত। এর জন্য প্রথমে আপনার এলাকার নিকটবর্তী থানায় একটি সাধারণ ডায়েরি (জিডি) করতে হবে। জিডির একটি কপি অবশ্যই নিজের কাছে রাখতে হবে। এরপর যেকোনো একটি দৈনিক পত্রিকায় বিজ্ঞপ্তি দিতে হবে। বিজ্ঞপ্তিতে নাম, শাখা, পরীক্ষার কেন্দ্র, রোল নম্বর, পাসের সাল, বোর্ডের নাম এবং কিভাবে আপনি সাটিফিকেট, নম্বরপত্র অথবা প্রবেশপত্র হারিয়েছেন তা সংক্ষেপে উল্লেখ করতে হবে।
এরপর শিক্ষা বোর্ডে
থানায় জিডি ও পত্রিকায় বিজ্ঞপ্তি প্রকাশের পর আপনাকে যেতে হবে যে বোর্ডের অধীনে পরীক্ষা দিয়েছেন সেই শিক্ষা বোর্ডে। শিক্ষাবোর্ডের ‘তথ্যসংগ্রহ কেন্দ্র’ থেকে আবেদনপত্র সংগ্রহের পর নির্ভুলভাবে পূরণ করতে হবে। এরপর নির্ধারিত ফি সোনালী ব্যাংকের ডিমান্ড ড্রাফটের মাধ্যমে বোর্ডের সচিব বরাবর জমা দিতে হবে। টাকা জমা হওয়ার পর আবেদন কার্যকর হবে। আবেদনপত্রের সঙ্গে মূল ব্যাংক ড্রাফট, পত্রিকায় বিজ্ঞপ্তির কাটিং ও থানার জিডির কপি জমা দিতে হবে।
আবেদনপত্রে যা পূরণ করতে হবে
আবেদনপত্র পূরণের ক্ষেত্রে প্রথমেই উল্লেখ করতে হবে আপনি কোন পরীক্ষার (মাধ্যমিক না উচ্চমাধ্যমিক) কী হারিয়েছেন এবং কী কারণে আবেদন করছেন। আবেদনপত্রের বিভিন্ন অংশে ইংরেজি বড় অক্ষরে এবং বাংলায় স্পষ্ট অক্ষরে পূর্ণ নাম, মাতার নাম, পিতার নাম, শিক্ষাপ্রতিষ্ঠানের নাম, রোল নম্বর, পাশের বিভাগ/জিপিএ, শাখা, রেজিস্ট্রেশন নম্বর, শিক্ষাবর্ষ এবং জন্মতারিখসহ বিভিন্ন তথ্য লিখতে হবে। পরবর্তী অংশে জাতীয়তা, বিজ্ঞপ্তি যে দৈনিক পত্রিকায় প্রকাশিত হয়েছে সেটির নাম ও তারিখ এবং সোনালী ব্যাংকের যে শাখায় ব্যাংক ড্রাফট করেছেন সে শাখার নাম, ড্রাফট নম্বর ও তারিখ উল্লেখ করতে হবে। আবেদনপত্রে প্রতিষ্ঠান প্রধানের সুপারিশের প্রয়োজন হবে। এতে তার দস্তখত ও নামসহ সিলমোহর থাকতে হবে। আর প্রাইভেট প্রার্থীদের আবেদনপত্র অবশ্যই গেজেটেড কর্মকর্তার স্বাক্ষর ও নামসহ সিলমোহর থাকতে হবে।
ব্যাতিক্রম আছে
নষ্ট হয়ে যাওয়া সনদপত্র/নম্বরপত্র/একাডেমিক ট্রান্সক্রিপ্টের অংশবিশেষ থাকলে পত্রিকায় বিজ্ঞপ্তি দিতে হবে না বা থানায় জিডি করতে হবে না। এ ক্ষেত্রে আবেদনপত্রের সঙ্গে ওই অংশবিশেষ জমা দিতে হবে। তবে সনদে ও নম্বরপত্রের অংশবিশেষে নাম, রোল নম্বর, কেন্দ্র, পাশের বিভাগ ও সন, জন্ম তারিখ ও পরীক্ষার নাম না থাকলে তা গ্রহণযোগ্য হবে না। আর বিদেশি নাগরিককে ব্যাংক ড্রাফটসহ নিজ সরকারের শিক্ষা মন্ত্রণালয়ের মাধ্যমে আবেদন করতে হবে।
কত টাকা লাগবে
সাময়িক সনদ, নম্বরপত্র, প্রবেশপত্র ফি (জরুরি ফিসহ) ১৩০ টাকা। এ ছাড়া ত্রি-নকলের জন্য ১৫০ টাকা এবং চৌ-নকলের জন্য ২৫০ টাকা ব্যাংক ড্রাফটের মাধ্যমে জমা দিতে হয়।

3^rd Step:Verify your Paypal:

Now you can verify in your paypal Bank account.Follow the step:

1.     1.   Log in your Payoneer account

2.       2. Then go to services->US Payment Service

3.      3.  Collect your Bank Routing no and account no.

4.     4.  Now signin your Paypal account and use your bank account no.

Now Paypal will send two small amount in your payoneer account within two days and after getting this money,you have to confirm to paypal.

Now your Paypal account is Bank verified.

Now you have a Verified Paypal Account.
